Care Coordination is built on a foundation of Microsoft Dynamics CRM 2013. This means that right out of the box there are a variety of features and functionality that you can use. In addition, we've built in a ton of fantastic functionality. I can't highlight it all now, but it is worth exploring 6 particularly awesome features.
1. Care Families
Also referred to as Cohorts, we provide functionality for integrating with your existing clinical systems to create care families and to manage patients within those care families. This enables you to create custom care plans, to automate the assignment of those care plans to specific care families, and to create control groups and pilot groups. There are other benefits as well, such as targeting Care Families with customized marketing campaigns, outreach, and education. As a result, we've created functionality that enables key staff to focus their time and energy on targeting key issues, goals and actions within each care family.
2. Care Planning
This is the heart of managing patients. Many ACOs have learned that managing the care for large groups of people can be…difficult. There are many players involved, a variety of clinical systems, and a seemingly exponential number of activities to manage. Our Care Planning system handles each of these extremely well. We have user and team based privileges and roles to enable differentiated user experiences for the WHOLE care planning team. We have the ability to integrate with pretty much any outward facing clinical system. We offer a variety of ways to get at that data, from windowing to patient context to full out integration. And finally, we have the full power of Microsoft Dynamics CRM activity management, enhanced by our own customizations for the specific needs of health care and care planning management.
3. Value-Based/ACO Contract Administration
Contracts are a native component of Microsoft Dynamics CRM. However, understanding and managing contracts in the context of value based ACOs is a whole different world. Our solution helps to answer questions like how to manage patients against a contract, how to manage clinical and performance based measures such as the 33 metrics from CMS, and compliance with contract-specific, patient-level requirements (hello Welcome Letter tracking!)
Gone are the days of impersonal patient care! By getting to know patients, their likes and dislikes, their needs and challenges, you have the ability to engineer the best possible set of interventions to help them be healthy. We have developed a personalization framework that enables you to ask your patients a series of related questions, and to create actionable steps and interventions that are tailored to their needs. If you are experienced in ACOs, then you know that a one size fits all approach does not work. Our personalization framework is a huge step towards real, impactful customization.

This was going to be a top 5 list, but alerts are so important, we had to grow the list! Anyone who manages health care data knows that there is just so darn much of it! Most organizations have a variety of data stores and clinical systems. Much of this information is key for effective patient management, but I've yet to meet someone in the health care space who wants to replicate any data more than necessary. As such, we've built an alerting engine inside Care Coordination. When something important or actionable happens in an external clinical system, we bring in just enough information to get the care team to work! And by integrating our customer Alert solution with native activity management, we have the ability to leverage the best of CRM, while also giving the care team the most useful tools possible.
