Tribridge Health360 Awarded 2016 Microsoft Health Innovation Award with Customer Dartmouth-Hitchcock

TAMPA - (April 13, 2016) - Tribridge has received a 2016 Microsoft Health Innovation Award for Tribridge Health360 and its implementation of the solution with Dartmouth-Hitchcock (D-H). Microsoft recognized Tribridge at the HIMSS16 Conference in the "Creating More Personal Computing" category, which emphasizes enhancing the consumer and clinician experience by making experiences mobile, easier, faster, more integrated and less prone to error or attack.

Health360 is a consumer-centered, cloud solution built on Microsoft Dynamics CRM online to enable healthcare providers to personalize care experiences and achieve population health goals. Dartmouth-Hitchcock is using Health360 as a key component of its ImagineCare offering to deliver personalized, coordinated care. ImagineCare combines Dartmouth-Hitchcock's evidence-based clinical pathways and behavior change research, Cortana Analytics Suite, Microsoft Dynamics CRM online, Azure and Tribridge Health360.

The Tribridge Health360 population management solution enables ImagineCare users to create and manage unique care plans for each patient. Through the solution, patients receive reminders, encouragement and dynamic updates to their care plans, which translates to better health, avoiding unnecessary trips to the emergency room and reducing the cost of care.
